#2- RCEB has identified a need for a specialty mental health clinic to serve underserved individuals with a co-occurring mental health support needs and intellectual or developmental disability in either Alameda or Contra Costa County.

 

#2- El RCEB identificó la necesidad de contar con un centro de salud mental especializado para atender a las personas desatendidas con discapacidades intelectuales o del desarrollo y que, a su vez, necesitan apoyo para la salud mental concurrente ya sea en el condado de Alameda o de Contra Costa.

 

#2- Tinukoy ng RCEB ang pangangailangan para sa isang espesyal na klinika sa kalusugang pangkaisipan upang maglingkod paglingkuran ang sa mga indibiduwal na kulang sa serbisyo na may magkasabay na mga pangangailangan sa suporta sa kalusugang pangkaisipan at kapansanan sa intelektwal o pag-unladsulong sa alinman sa Alameda o Contra Costa County.

#3- RCEB is seeking a service provider to operate a Level 7 facility to serve 4 children (17 years and under) who may have severe behaviors, may not be adequately served in a Level 6 facility, and/or stepping down from a Children’s Community Crisis Home (CCH) or Stabilization Therapeutic Assistance Reintegration Home (STAR). The selected Level 7 service provider will provide a highly structured environment with close supervision by staff and with consultants beyond what a Level 6 facility is required to provide. The identified clients for this home may have severe behaviors such as PICA, elopement, verbal aggression, property damage and self-injurious behaviors.

 

#3- El RCEB está buscando un proveedor de servicios para dirigir una instalación de nivel 7 y atender a 4 niños (de 17 años y menores) que pueden tener comportamientos severos, que pueden requerir cuidados no atendidos de forma adecuada en una instalación de nivel 6 o derivados de un Centro comunitario para niños en crisis (Children’s Community Crisis Home, CCH) o de un Centro de asistencia terapéutica para la estabilización y reinserción (Stabilization Therapeutic Assistance Reintegration Home, STAR). El proveedor de servicios de nivel 7 seleccionado brindará un entorno altamente estructurado supervisado minuciosamente por el personal y con asesores que superen los servicios requeridos para una institución de nivel 6. Los clientes identificados para este hogar pueden tener comportamientos severos como el PICA, fugas, agresión verbal, daños a la propiedad y conductas de autolesión.

Nhà cung cấp dịch vụ Cấp 7 được chọn sẽ cung cấp một môi trường có cấu trúc chặt chẽ với sự giám sát chặt chẽ của đội ngũ nhân viên và tư vấn viên vượt xa yêu cầu cung cấp của cơ sở Cấp 6. #3- RCEB 现寻求一家服务供应商，负责运营一处第 7 级设施，为 4 名（17 岁及以下）可能存在严重行为问题的儿童提供服务。这些儿童可能在第 6 级设施中无法获得充分支持，或正从儿童社区危机之家 (CCH) 或稳定治疗协助转衔适应之家 (STAR) 过渡。选定的第 7 级服务供应商应提供高度结构化的生活环境，由工作人员对入住儿童实施密切监督，配备的顾问支持应超出第 6 级设施要求。拟入住本设施的儿童可能存在严重行为问题，比如：异食症 (PICA)、逃逸、言语攻击、财物破坏及自伤行为。 由于本项目不涉及房屋开发机构，选定的服务供应商必须在阿拉米达县或康特拉科斯塔县拥有一处住宅。该住宅须设有 4 间单人卧室，具备全面的无障碍设计，能接纳行动无 – 2 – 法自理的服务对象，并能向社区照护牌照颁发机构申请持牌。选定供应商须承诺运营该设施至少 10 年。

 

 

#4 REPOST- RCEB has identified a need for a short-term (3-5 day) emergency shelter to provide safe, accessible and immediate sheltering options for individuals served by RCEB during disasters. The shelter needs to provide supports for 25 individuals with intensive medical needs who might otherwise be unserved or underserved in a traditional congregate shelter setting.

 

#4 REPOST – El RCEB identificó la necesidad de contar con un albergue de emergencia a corto plazo (3 a 5 días) para brindar opciones de albergue seguras, accesibles e inmediatas para las personas atendidas por el RCEB durante desastres. El albergue debe poder proporcionar apoyo a 25 personas con necesidades médicas intensivas que, de no contar con este albergue, podrían quedar desatendidas o ser mal atendidas en el entorno de un albergue tradicional.

#5 – RCEB has identified a need for a service provider to take over operation of an existing Enhanced Behavior Support Home (EBSH) serving four (4) individuals with I/DD who also have forensic, behavioral and/or mental health support needs. The home is located in Antioch, California.

 

#5- RCEB ha identificado datos necesidad para Un proveedor de servicios se hará cargo de la operación de un Hogar de Apoyo Conductual Mejorado (EBSH) existente, que atiende a cuatro (4) personas con discapacidades intelectuales y del desarrollo (DI/DD) que también necesitan apoyo forense, conductual o de salud mental. El hogar está ubicado en Antioch, California.

 

#5- RCEB has identifieda kailangan para sa isang tagapagbigay ng serbisyo ang siyang mamamahala sa pagpapatakbo ng isang umiiral na Enhanced Behavior Support Home (EBSH) na nagsisilbi sa apat (4) na indibidwal na may I/DD na mayroon ding mga pangangailangan sa forensic, behavioral at/o mental health support. Ang tahanan ay matatagpuan sa Antioch, California.

#6 REPOST – RCEB has identified a need for a service provider to take over the operation of an existing Community Crisis Home (CCH) for adults. An adult’s CCH means an adult residential facility certified by the California Department of Developmental Services (DDS) and licensed by the California Department of Social Services (DSS) that provides 24-hour nonmedical care to individuals with developmental disabilities receiving regional center services and in need of crisis intervention services.

El CCH para adultos es un centro residencial para adultos certificado por el Departamento de Servicios de Desarrollo de California (DDS) y autorizado por el Departamento de Servicios Sociales de California (DSS), que brinda atención no médica las 24 horas a personas con discapacidades del desarrollo que reciben servicios del centro regional y necesitan servicios de intervención en crisis.

  • San Andreas Regional Center is seeking a housing access service provider who would be working with Alta California Regional Center’s housing service provider, On My Own, in using the On My Own roommate matching database for Deaf and Hard of Hearing individuals. The selected housing service access vendor would be providing supports to individuals served by San Andreas Regional Center, Golden Gate Regional Center, and Regional Center of the East Bay.

Self-Determination Local Advisory Committee Projects

RCEB’s Self-Determination Local Advisory Committee has prioritized the needs in the local area for the use of the provision of funds and is seeking proposals from interested parties to carry out one or more of the projects which the committee wishes to fund.
