Everyone selected for this program must attend an orientation, so if you were selected we would like to invite you to attend one of the orientations scheduled in the upcoming months.

  • Session #11: San Leandro Office – 02/28/20, 9am–3pm
  • Session #12: Concord Office – 03/13/20, 9am–3pm
  • Session #13: San Leandro Office – 03/30/20, 9am–3pm

Please let your case manager know if you would like to attend one of the scheduled orientations. If you are no longer interested in participating in the Self-Determination Program, please let us know.

If you signed up for SDP and were not selected, your name will remain on the list with DDS. If selected individuals decide they do not want to participate in SDP, DDS will select additional names from the list. If your name is not on the list for SDP and you would like to sign up, please view the PowerPoint below and then click on the survey link at the end of it to sign up for SDP.  If you have not yet received written notification about being selected, you can check your status on DDS’s website to enter your Unique Client Identifier (UCI) number. In October of 2013, Governor Edmund G. Brown Jr. signed into law the Self-Determination Program, which will provide consumers and their families with more freedom, control, and responsibility in choosing services and supports to help them meet objectives in their Individual Program Plan. 

In the Welfare and Institutions Code Section 4685.8, the services and supports will be directed by a participant through person-centered planning, as they are designed to assist the participant to achieve personally defined outcomes in community settings that promote inclusion.
